Dr. Dan Simon and Dr. Don Baim present a challenging chronic total occlusion (CTO) case. The doctors discuss the challenges faced by interventional cardiologists when treating patients with CTOs, and they present the characteristics that interventional devices must contain for these challenges to be overcome. As part of the GREAT trial, an ILT SafeCross wire is utilized in the treatment of a 47 year old male patient with a chronically occluded right coronary artery after attempts with conventional wires were unsuccessful. The doctors demonstrate how the SafeCross wire uses a fiberoptic tip to help prevent perforations during the treatment of CTOs. Guide catheter selection, advancement technique, and steering issues are discussed during the case.
